This interview form came from the Critical Inventory. It is best for parents who know their child is having difficulties with literacy. It does not include items to gather health information.
Parent/Guardian Interview Form
This interview form should be used to background information in three categories: general information (home & health), school information (i.e. history, services, etc.), and literacy information (i.e., access to texts, literacy engagement, etc.).
